This Privacy Policy governs your access to and use of MyDDSLab mobile application, including any content, functionality and services offered on or through MyDDSLab mobile application (the “Application”).

When you access the Application, the Company will learn certain information about you during your visit.
Information You Provide to Us. The Application provides a place for users to provide information. We collect information that users provide by filling out forms on the Application, communicating with us via contact forms, responding to surveys, providing comments or other feedback, and providing information when ordering a product or service via the Application. We use information you provide to us to deliver the requested product and/or service, to improve our overall performance.
HOW DO WE USE THE INFORMATION THAT YOU PROVIDE TO US?
We use personal information for purposes of presenting our Application and its contents to you, providing you with information, and/or services, administering our business activities, and providing customer service. The Company may use software programs to create summary statistics, which are used for such purposes as assessing the number of visitors to our site, determining technical design specifications, and identifying system performance or problem areas.
DISCLOSURE OF YOUR INFORMATION
We do not sell, rent, lease or otherwise transfer any information collected whether automatically or through your voluntary action. These MyDDSLab™ Terms of Service apply to your use of the MyDDSLab™ online service, related customer and patient data storage and associated websites (collectively, “MyDDSLab”). In these MyDDSLab Terms of Service, DDS Lab, Inc. is referred to as “DDS” or “we” or “our”. There are a number of legal issues that pertain to our provision and your use of MyDDSLab. Please take the time to read through these MyDDSLab Terms of Service so that you fully understand our intent in making MyDDSLab available to you and your responsibilities with respect to its use.
By clicking “Accept”, you agree that your use of MyDDSLab is governed by the terms and conditions set forth in these MyDDSLab Terms of Service, including any terms, policies or guidelines incorporated herein by reference (collectively, this “Agreement”), and you accept and agree to be bound by this Agreement. This Agreement is a legally binding agreement between you and DDS. If you are registering to use MyDDSLab on behalf of an organization or other legal entity, you represent that you have the authority to bind the legal entity, and that the legal entity is duly organized, validly existing and in good standing. You further agree that the legal entity will be bound by and responsible for any breach of this Agreement. If you do not agree to all the terms and conditions of this Agreement, then you may not access MyDDSLab or use any related services, you should not click “Accept”, and you should exit the site immediately. If these terms and conditions are considered an offer by DDS, acceptance is expressly limited to these terms.
